Does UniSuper require its own form?

YES for the classic information-access TPA - UniSuper's own 'Third party authority form' (UNISF00217) is the required instrument; no indication anywhere that a generic signed letter is accepted, and the adviser-support page directs advisers specifically to this form. HOWEVER UniSuper now also runs a fully digital alternative for advisers: the enduring 'Financial Adviser Authority' (FAA) initiated inside Adviser Online and approved by the client in their Member Online inbox - no paper at all. So the practical answer is: UniSuper form (paper/upload) OR digital FAA via portal; generic letters unverified/assume rejected.

Form details

Form: 'Third party authority form', code UNISF00217 0325, issue date 23 April 2026 (current), at https://www.unisuper.com.au/-/media/files/forms/change-details-and-manage-account/third-party-authority-form.pdf. Read in full: member completes in black/blue ballpoint pen OR completes the equivalent online via their Member Online account (Manage Account page) - a genuinely digital member-side path. Requires member number, DOB, member signature + date, AND the nominated person's signature (Section 2) - the adviser must counter-sign, a detail that catches practices out. Financial advisers MUST provide their AFSL number ('You must provide an Australian Financial Services Licence (AFSL) number if you choose financial adviser'). Optional tick box extends authority to 'any representative of the nominated company or business' - tick it to cover the whole practice. Validity: 24 months from signing, not effective until UniSuper receives the form; expires automatically requiring a fresh authority. Information-only: no transactions, no detail changes, and TFN/bank details never released. UniSuper can cancel unilaterally, including if the adviser's AFSL is suspended/cancelled. Cancellation requests process in 1-3 business days. No ID certification requirement on the form. On acceptance the adviser is emailed a copy of the client's Member Summary Report (per adviser-support page).

Where and how to lodge

Three channels: (1) Email to enquiry@unisuper.com.au with subject line 'Third party authority request' - this is the channel UniSuper's adviser-support page itself tells advisers to use, with receipt confirmed within 2 business days (page content is JS-rendered; wording verified via two independent search-index captures of unisuper.com.au/contact-us/adviser-support). (2) 'Upload a document' tool at unisuper.com.au/contact-us (stated on the form itself as the electronic return channel). (3) Post to UniSuper, Level 1, 385 Bourke Street, Melbourne VIC 3000. No fax. Alternatively the member can do the whole thing digitally in Member Online, or the adviser can skip the TPA entirely by sending a digital FAA request from Adviser Online. Fastest-processed channel not published; the digital FAA is structurally fastest since consent is captured in-system.

Published turnaround

Published micro-standards only: receipt of an emailed TPA form confirmed within 2 business days (adviser-support page); authority cancellations processed in 1-3 business days (on the form). No published end-to-end TPA processing SLA - unverified. Complaints standards are published (see IDR).

Watch-outs

(1) 24-month hard expiry on the paper TPA - authorities silently lapse and 'the nominated third party will not be able to obtain information... until a new authority has been provided'; a renewal-tracking feature sells itself here. (2) The nominated person must SIGN the form (Section 2) as well as the member - forms lodged with member signature only are incomplete. (3) Missing AFSL number = mandatory-field failure for adviser nominations. (4) UniSuper can kill an authority unilaterally if the adviser's AFSL is suspended/cancelled. (5) Tick the company/business authorisation box or the authority binds to one named individual - staff turnover then breaks access. (6) Digital FAA prerequisite trap: client must have Member Online AND identity verified within the past 12 months, or the FAA request cannot be actioned - the paper TPA is then the only path. (7) Two-authority confusion: TPA (24 months, info-only) vs FAA (enduring, adviser-linked) are separate regimes; revoking one does not obviously revoke the other. (8) Authority 'won't take effect until we've received your completed form' - post is slow, use email/upload. (9) Adviser Online had scheduled-maintenance downtime noted on its own support page at fetch time (Aug 2026) - portal availability is not guaranteed. No widespread adviser-forum complaints about UniSuper TPA turnaround found (member phone support generally praised on Whirlpool/ProductReview) - unverified whether TPA processing is genuinely fast; the 2-business-day receipt confirmation is the only published number.

No complaints email published - lodge via: online complaint form at unisuper.com.au/contact-us/complaints; phone 1800 331 685 (8:30am-6:00pm Melbourne, Mon-Fri; intl +61 3 8831 7901); or post to Complaints Officer, UniSuper, Level 1, 385 Bourke St, Melbourne VIC 3000. Published response commitments: 45 days for super complaints, 30 days for financial advice complaints, 90 days for objections to death benefit distributions; written notification if they cannot meet the timeframe. Under s1017C of the Corporations Act, a trustee must make reasonable efforts to comply with an information request within one month. If UniSuper exceeds its own published standard, a formal internal-dispute-resolution complaint starts a 45-calendar-day clock under ASIC Regulatory Guide 271 — with AFCA as the next stop.
