Does Rest Super require its own form?

Rest's published third-party representative page states it accepts EITHER the prescribed 'Rest Third Party Authority form' OR a generic written authority, provided the written authority contains: member full name, member date of birth, member number, member signature and date, plus the full name and address of the company/solicitor if completed by a company or solicitor. Signature: member must sign AND date — the form states it is invalid without both; it is designed to be scanned and emailed (wet signature scanned; no statement that typed/e-signatures are accepted — unverified). No ID certification required to lodge the form itself, but third-party reps handling claims must meet Rest's Proof of Identity standards, and Rest/its insurer may run identity checks on the nominated rep before disclosing claim information. Watch-outs

(1) Default 12-month expiry: unless the member ticks 'valid until revoked in writing', the authority silently lapses 12 months after signing — a prime cause of advisers discovering mid-engagement that Rest won't talk to them; SuperChase should capture which term box was ticked and diarise expiry. (2) One-nomination-at-a-time: the form's declaration states the new nominated representative REPLACES any existing third-party nominations — lodging a TPA can inadvertently wipe another authorised party (e.g. the client's accountant or lawyer). (4) Information-only: no transaction or detail-change power, so every action still requires the member. (6) Named absent from the April 2026 FAAA/Chant West adviser-ready list, and no external advice fee deduction from accounts — adviser servicing is not a strategic priority, consistent with reported industry-fund adviser frustrations (lost authorities, no adviser help lines).
