Does Insignia Financial — MLC MasterKey require its own form?

Own forms in practice, and they differ by product. MLC MasterKey: adviser appointment/information access requires MLC's client-signed 'Change of financial adviser' form (currently linked on the adviser forms page); a separate 'authorised representative' (info-only third party) is added by the MEMBER online after login ('Update authorised person'), not via a downloadable TPA form. MLC's adviser forms page states forms not on its e-signature list 'will need to be completed with a handwritten signature before we can accept it' — no published acceptance of a generic signed authority letter (generic-letter acceptance: unverified, assume own form required). Requires policy number(s) (up to 6 per form), member personal details, new adviser name/licensee/division & adviser number, client (policy owner) signature(s). Effect: replaces the existing adviser — old adviser loses remuneration AND information access; new adviser gains info access. E-signature by the CLIENT is accepted only via MLC-approved providers (DocuSign preferred; also Adobe Sign, Dropbox Sign/HelloSign, Xplan, PleaseSign, FuseSign, SignNow, PandaDoc, MYOB, Xodo Sign, Annature, Conga Sign) and MUST include the provider's certificate of completion with an email address matching MLC records. MLC explicitly will NOT accept forms e-signed by third-party advisers or POAs. POA documents must be certified copies sent by MAIL only (email/fax refused). Email is the practical channel for both; no published statement of which channel is processed fastest (unverified).

Published turnaround

No published turnaround/service standard for third-party information requests on either product (unverified). MLC provides request-status visibility via My Work Tracker in AdviserOnline rather than a published SLA.

Watch-outs

1) MLC will not accept forms e-signed by third-party advisers or POAs — the client personally must e-sign via an approved vendor, and the certificate of completion email must match MLC's records, or the form bounces. 2) Any form not on MLC's e-signature list requires a handwritten signature. 3) POA documents are mail-only (certified copies); emailed/faxed POAs are rejected — a notorious slow path. 4) The MasterKey 'Change of financial adviser' form is a full adviser replacement (kills the old adviser's access and remuneration) — there is no lighter 'information-only' adviser TPA form, so one-off info requests are awkward. 5) The Nov-2024 separation from NAB changed processes: advisers can no longer update client email/mobile in AdviserOnline (contact-centre only), and login/2FA changes tripped up users. 6) Industry press (Money Management, ifa) documents adviser frustration and $1.5bn FY24 platform outflows driven by the MLC Wrap/Navigator→Expand migration (completed March 2024, $39bn FUA / 94,000 accounts) — advisers 'lost patience' with service delays during migration; service reputation is a live issue for Insignia. 7) mlc.com.au aggressively blocks non-browser traffic (Akamai), so automated form-fetching for MLC needs a real browser context — relevant to SuperChase's implementation.

MLC's complaints page commits to a written response within 'the relevant maximum timeframe' (RG 271: 45 calendar days for super complaints) without stating the number on-page. Both escalate to AFCA (afca.org.au, 1800 931 678, info@afca.org.au, GPO Box 3 Melbourne VIC 3001); death-benefit objections must reach AFCA within 28 days of the written decision.

Under s1017C of the Corporations Act, a trustee must make reasonable efforts to comply with an information request within one month.
