Does Cbus Super require its own form?

Effectively yes for paper lodgement. Cbus publishes its own 'Third party authority' form and, on its For Advisers page, lists the accepted routes as: digital TPA via Adviser Online, the Cbus TPA PDF returned by email, or power of attorney documents by post. No published statement that a generic signed authority letter is accepted — treat generic letters as not accepted (unverified whether they are processed in practice).

Form: 'Third party authority', code 1239.4 06-23 ISS5 (issue 5, June 2023 — still the current published version as of Aug 2026), 2 pages. Requires: Super member number and/or Super Income Stream member number, DOB, personal details. Professional representative section (Step 3b): company/practice name, nominated person, services provided (financial/legal/other), AFSL/ABN, option to nominate all staff of the practice, and a question on whether the practice participates in the Cbus–FAAA Member Referral Program. Signature: member signs and dates; no ID certification required with the form, but the nominated third party must pass a phone security check before information is released. Expiry: valid TWO YEARS from the date the form is RECEIVED by Cbus (not signing date); renewal requires a completely new form; member can cancel any time by phone or in writing. Published turnaround

Digital TPA via Adviser Online: processed instantly after client SMS/email confirmation (published). Paper TPA: written confirmation 'once it has been received' — no timeframe published (unverified). No published service standard for general third-party information requests. Privacy complaints: 30-day response commitment.

Watch-outs

(1) Documented systemic delay history — the strongest escalate-early signal in the library: ASIC sued the trustee in Nov 2024 (24-251MR) and the Federal Court ordered a $23.5M penalty in 2025 (25-286MR) for serious failures processing death benefits and TPD claims — 7,000+ members affected; in Mar–May 2023, 48–56% of death claims and 38–43% of TPD claims had been pending over 365 days; APRA separately took a court-enforceable undertaking and opened an investigation. Failures included poor oversight of the contracted administrator (AAS/MUFG — the same administrator operating the adviser portal), so claims-adjacent correspondence should be assumed slow and diarised for IDR escalation. (2) Portal TPA rejections: member details must be up to date BEFORE lodging — if Cbus holds a stale mobile/email, the client SMS/email confirmation cannot complete. (3) 2-year validity runs from the date Cbus RECEIVES the authority, and renewal requires a whole new form — no simple extension. (4) The nominated third party must pass a phone security check on every call and can never access the TFN — advisers report friction completing ATO/rollover tasks that need it. (5) Information-only authority — no transactions — though one-off advice fee deduction is separately available through the portal. (6) Did not meet the April 2026 FAAA/Chant West adviser-ready accreditation despite the portal and its FAAA referral program.
