For becoming the servicing adviser (which is how advisers actually get ongoing access), AMP mandates its OWN form: 'Authority to change your financial adviser' (form 05260.1_F, 13 May 2023), issued by NM Super and NMMT and explicitly covering AMP Super Fund (SignatureSuper), Wealth Personal Super & Pension Fund (MyNorth super/pension) and MyNorth/North IDPS — verified by reading the form. For a pure information-only third-party authority, AMP publishes NO super-specific TPA form; whether a generic member-signed letter of authority is accepted for SignatureSuper/North info requests is UNVERIFIED (industry practice suggests yes via ampsuper@amp.com.au / north@amp.com.au, but AMP publishes nothing). WARNING: the 'Third Party Access – Personal' (304054) and 'Third Party Access – Company' (NS7970) forms that surface in searches are AMP BANK forms — they do not apply to super.

No ID certification stated. No expiry — servicing rights persist until changed or removed (removal is by PHONE to 131 267 only; no removal form). Separate tick-boxes to transfer servicing rights and to transfer advice fee arrangements — if the fee section is blank, existing fee arrangements do NOT transfer, and any new fee agreement supersedes the old one. The form warns the new adviser may not be assignable on employer super plans where an adviser is already assigned, or on credit products the licensee can't advise on.

Watch-outs
